The zip code you enter will be used to determine the closest drug testing center where you will go to take your test. A donor pass/registration form with the local testing center address,hours of operation and instructions will be sent to the e-mail address you provided. Take this form with you or have available on your smart phone to provide to the testing center. No appointment is necessary in most cases.
USCG Consortium Enrollment-If you have selected the USCG Consortium, you will receive a certificate of enrollment via email for immediate proof of compliance.

• Not all testing centers listed are certified/available to perform all tests. An alternate location will be selected if required. If the alternate location is more than 15 miles from the zip code you provided, you will be contacted prior to processing your order confirmation.
• When your order is processed you will receive a payment/transaction receipt & then a donor/registration pass with the location and testing information. Please have the donor pass available at the collection/testing location (not the transaction receipt)
